Social work services can support you and your family as you adjust to life after injury, illness or disability.

Below are some of the specific areas in which our social workers may be able to provide support with:

– Psychosocial assessment of impacts of illness or disability on your life

– Adjustment support and counselling related to life changes

– Engagement with complex systems (e.g. Centrelink, housing, NDIS) with an advocate and experienced systemic navigator to help maximise your outcomes and provide you with education and coaching as we go so that you can also learn the skills to navigate these systems

– Medicolegal reports regarding the emotional, social, psychological and environmental impact of injury.

Care Rehab social workers use an eclectic approach to ensure you achieve your goals. You can expect to have access to a skilled and dedicated social worker who will work with you to identify your needs and develop a plan forward with you and your family.

Social Work services are available in your home or virtually using videoconference technology. Care Rehab provides a national virtual service and a travelling service where required.
